Therapy Practice Website
Conscious Path Therapy Website Project
Conscious Path Therapy is a Folsom therapy practice offering individual therapy, EFT couples therapy and parent support. The practice had no website at all, and this marriage and family therapist website gave it a full online home, with in-depth online intake forms and business cards to match.

Online intake forms and business cards to match

In-depth online intake forms
A new client intake form and a separate couples questionnaire collect history, goals and relationship details before the first session, so appointments start with context instead of clipboards. Both forms are linked from the homepage, the about page and the main menu.

Therapist business cards
Cards designed with the same logo, colors and type as the website, so referrals and in-person introductions point to a brand clients will recognize when they look the practice up. The layout stays simple, with the green round logo and only the details someone needs to call or book.
Strategic Direction & Design Intent
A marriage and family therapist website built around a calm first step
People usually look for a therapist during a hard stretch. The site needed to feel calm and grounded, so someone landing on it could tell right away they had found the right kind of help.
Starting Point
The practice had no website. Clients found it through referrals and directories, with nowhere to learn about services, read about the therapist's approach or start intake online, and nothing came up when people searched for a therapist in Folsom.
Project Goals
Introduce the practice and its three core services, explain the EFT couples therapy approach in plain language and move intake paperwork out of the office and onto the website.
The Challenge
Couples therapy intake asks a lot of personal questions. The forms had to be detailed enough to be useful in session while still feeling approachable for someone filling them out for the first time.
The Approach
Soft greens, nature imagery and open spacing reflect the practice's mind, body and spirit focus. The new client intake form and couples questionnaire sit right in the main menu, so new clients never have to hunt for them. Each service on the homepage links straight to the services page, and the about page pairs the therapist's credentials with a direct link to the intake form.
The Result
The finished Conscious Path Therapy website

The Outcome
The Folsom therapy practice now has a full online home. Visitors can explore individual therapy, EFT couples therapy and parent support, read about the therapist's 13 years of private practice experience and complete intake before they ever walk in the door. For couples, the questionnaire means the first session can focus on the relationship instead of paperwork. For the therapist, it means less admin time and a clearer picture of each client from the start.
